Overview

Systemic lupus erythematosus, usually called lupus or SLE, is a condition in which the immune system loses its normal ability to tell self from non-self and begins to target healthy tissues. Because those tissues are spread throughout the body, lupus is described as a multi-system disease: it can involve the skin, joints, kidneys, blood cells, heart, lungs, and nervous system, though rarely all at once. It affects people of any age but is far more common in women, particularly during the reproductive years, and shows notable differences in frequency and severity across ancestral backgrounds. The Underlying Biology

Lupus is characterized by a loss of immune tolerance to nuclear self-antigens, driving production of autoantibodies such as antinuclear antibodies (ANA), anti-double-stranded DNA (anti-dsDNA), and anti-Smith. These autoantibodies combine with self-antigens to form immune complexes that deposit in tissues like the kidney's glomeruli, activating the complement cascade and consuming C3 and C4. A central mechanism is impaired clearance of apoptotic cellular debris, which exposes nucleic acids that engage toll-like receptors and drive the type I interferon signature seen in many patients. B cells produce autoantibodies while dysregulated T cells and cytokines, including interferon-alpha, BAFF, IL-6, and TNF, sustain the inflammatory loop. Genetic susceptibility, involving HLA and complement-related genes, interacts with hormonal influences and environmental triggers such as ultraviolet light, certain infections, and oxidative stress. Neutrophil extracellular traps and mitochondrial signals further amplify the response. Clinically, this biology is tracked through ANA and specific autoantibody panels, complement levels, complete blood counts that may reveal cytopenias, inflammatory markers, and urinalysis for kidney involvement. The result is a fluctuating disease driven by innate and adaptive immunity operating together.
