How It Works

Vitamin D starts as a nutrient you make in your skin from sunlight or take in from a few foods, but it does not stay a simple vitamin. The body activates it through two steps, in the liver and then the kidney, into a hormone that circulates and delivers instructions to cells almost everywhere. In everyday terms, this activated form acts like a chemical messenger that helps the gut absorb calcium, tells bone-building cells when to work, and helps tune immune responses. Because receptors for it appear in bone, muscle, immune cells, and many other tissues, researchers study vitamin D far beyond bones alone. The Science & Mechanism

Vitamin D exists mainly as D3 (cholecalciferol), made in skin when UVB light converts 7-dehydrocholesterol, and D2 (ergocalciferol) from some foods. Neither form is active on its own. The liver first hydroxylates it to 25-hydroxyvitamin D (calcidiol), the main circulating store measured on labs, and the kidney then converts that to 1,25-dihydroxyvitamin D (calcitriol), the biologically active hormone. Calcitriol binds the vitamin D receptor (VDR), a nuclear receptor that partners with the retinoid X receptor and binds vitamin D response elements in DNA, regulating transcription of hundreds of genes. Its best-characterized job is mineral homeostasis: it increases intestinal absorption of calcium and phosphate and works with parathyroid hormone to maintain serum calcium, supporting bone mineralization and remodeling. VDR is also expressed in immune cells, muscle, and other tissues, where research explores roles in innate and adaptive immunity, insulin sensitivity, and cell differentiation. Production of the active hormone is tightly regulated by parathyroid hormone, calcium, phosphate, and the hormone FGF23, and levels are influenced by sun exposure, skin pigmentation, latitude, age, adiposity, and kidney function.
