IGF-1, or insulin-like growth factor 1, is a hormone the body produces largely in the liver after growth hormone signals it to do so. In everyday terms, growth hormone is the message and IGF-1 is much of the action: it travels through the bloodstream and tells cells to grow, divide, and repair themselves. It is called insulin-like because its structure resembles insulin and it shares some overlapping effects on how cells take up nutrients. The Science & Mechanism
IGF-1 is a single-chain polypeptide hormone whose production is driven mainly by pituitary growth hormone (GH) acting on hepatic GH receptors, with additional autocrine and paracrine production in many peripheral tissues. Once secreted, most circulating IGF-1 is bound to IGF-binding proteins, predominantly IGFBP-3, and stabilized in a ternary complex with the acid-labile subunit, which extends its half-life and buffers its availability. IGF-1 acts primarily through the IGF-1 receptor, a transmembrane tyrosine kinase that, upon binding, activates the PI3K-Akt and Ras-MAPK cascades to promote cell growth, protein synthesis, proliferation, and survival while inhibiting apoptosis. It also participates in a negative-feedback loop, signaling the hypothalamus and pituitary to restrain further GH release. Because IGF-1 mediates much of GH's anabolic effect on bone, muscle, cartilage, and other tissues, it is frequently measured as an integrated marker of GH status.
