Thyroid antibodies are proteins made by the immune system that, in some people, target the thyroid gland instead of an outside threat. The two most familiar are anti-thyroid peroxidase (TPO) antibodies and anti-thyroglobulin (Tg) antibodies, both aimed at components the thyroid uses to build its hormones. In everyday terms, checking these markers is like looking for the immune system's fingerprints on the thyroid, which can help explain why the gland is behaving the way it is. Their presence is associated with autoimmune thyroid conditions and can appear even before standard thyroid numbers shift. The Science & Mechanism
Thyroid peroxidase (TPO) and thyroglobulin (Tg) are essential to normal thyroid hormone synthesis: TPO is the enzyme that oxidizes and organifies iodide onto tyrosine residues of thyroglobulin, the large scaffold protein on which thyroxine (T4) and triiodothyronine (T3) are assembled and stored within the follicular colloid. In autoimmune thyroid disease, immune tolerance to these self-antigens is lost, and B cells produce autoantibodies against TPO and Tg. Anti-TPO antibodies are the most sensitive serologic marker and are detectable in the large majority of Hashimoto thyroiditis cases, where lymphocytic infiltration and follicular damage are described; anti-Tg antibodies appear in a substantial subset and add information when TPO is negative. Research characterizes these antibodies largely as markers of an underlying cell-mediated process rather than the primary drivers of tissue injury. They are distinct from TSH-receptor antibodies (TRAb), which stimulate the receptor in Graves disease. Studies also examine how antibody presence is associated with future changes in thyroid function, variability across pregnancy, and their limited role in monitoring once a diagnosis is established.
