Free T3 is the small, unbound portion of triiodothyronine circulating in your blood, and it is considered the most metabolically active thyroid hormone. Most thyroid hormone travels attached to carrier proteins, but only the free fraction can enter cells and switch on thyroid signaling. In everyday terms, if the thyroid axis is a thermostat for metabolism, free T3 is the signal that actually reaches the furnace. The body makes most of its T3 not directly from the thyroid gland but by converting T4 into T3 in tissues like the liver. The Science & Mechanism
Triiodothyronine (T3) is the higher-affinity ligand for nuclear thyroid hormone receptors (TR-alpha and TR-beta), which act as ligand-activated transcription factors regulating genes involved in metabolism, thermogenesis, and growth. Only about 0.3 percent of total T3 circulates unbound; the rest is reversibly bound to thyroxine-binding globulin, transthyretin, and albumin. Because bound hormone is not receptor-available, the free fraction is the physiologically relevant measure. Roughly 80 percent of circulating T3 is produced outside the thyroid, generated when the prohormone T4 is deiodinated by selenium-dependent iodothyronine deiodinases (DIO1 and DIO2) that remove an outer-ring iodine. The counter-regulatory enzyme DIO3 inactivates hormone by forming reverse T3. Once inside the cell, free T3 binds its receptor, which partners with retinoid X receptor on thyroid response elements to modulate transcription, influencing mitochondrial activity, basal metabolic rate, cardiac output, and lipid handling. Research explores how conversion efficiency, nutrient cofactors such as selenium and iron, illness, and caloric status can shift T3 availability independently of T4, which is why free T3 is studied as a window into tissue-level thyroid activity.
