How It Works

Vitamin D3 is the form of vitamin D your skin makes when exposed to sunlight, and the same form found in many foods and supplements. Once in the body, it is converted in two steps into an active hormone that tells the gut to absorb calcium and helps keep bones and muscles working properly. Beyond bone, receptors for active vitamin D are found throughout the body, including in immune cells, which is why research explores its broader role in everyday health. Because sun exposure, skin tone, latitude, and age all affect how much a person makes, blood levels differ dramatically from one individual to the next. The Science & Mechanism

Vitamin D3 (cholecalciferol) is a secosteroid that functions less like a classic vitamin and more like a prohormone. After it is synthesized in the skin from 7-dehydrocholesterol or obtained from the diet, it undergoes two hydroxylation steps: the liver converts it to 25-hydroxyvitamin D (calcifediol), the main circulating form measured on labs, and the kidney converts that to 1,25-dihydroxyvitamin D (calcitriol), the biologically active hormone. Calcitriol binds the vitamin D receptor (VDR), a nuclear receptor that partners with the retinoid X receptor to bind vitamin D response elements in DNA and regulate transcription of hundreds of genes. Classically this promotes intestinal absorption of calcium and phosphate and, with parathyroid hormone, supports bone mineralization and remodeling. Because the VDR is expressed in many tissues, including monocytes, macrophages, and T and B lymphocytes, research also describes roles in innate and adaptive immune signaling, such as induction of antimicrobial peptides. Many cells can locally convert calcifediol to calcitriol, which is thought to underlie these tissue-specific, non-skeletal effects that ongoing research continues to characterize.
