Pentosan polysulfate is a semi-synthetic, highly sulfated polysaccharide prepared from beech-wood xylan and structurally related to the body's glycosaminoglycans, giving it heparin-like characteristics. In cartilage and joint biology, research describes several converging actions. It is studied for inhibiting catabolic enzymes such as matrix metalloproteinases (MMPs) and aggrecanases that break down cartilage proteoglycans and collagen, and for reducing pro-inflammatory mediators including certain cytokines and complement activation. At the same time, laboratory work explores its ability to stimulate chondrocytes and synovial cells to produce hyaluronic acid and proteoglycans, components that support the resilience and lubrication of joint tissue. Its polyanionic, heparin-like structure is also associated with fibrinolytic and mild anticoagulant activity, which research examines in relation to subchondral bone circulation and clearance of debris within the joint.
