How It Works
Testosterone is a steroid hormone that acts as the body's main androgen, meaning it drives many of the traits associated with male physiology, though it is important in every body. In men it is produced mostly by the Leydig cells of the testes; in women, smaller but meaningful amounts come from the ovaries and adrenal glands. It travels through the bloodstream, mostly bound to carrier proteins, and only the free, unbound portion can enter cells and signal. Once inside, it influences muscle and bone, red blood cell production, libido, mood, and energy, and some of it is converted into other active hormones the body also relies on. Levels naturally shift with age, sleep, stress, body composition, and overall health, which is why they are best interpreted in context.
The Science & Mechanism
Testosterone is synthesized from cholesterol through a series of steroidogenic enzymes, primarily in testicular Leydig cells under the control of luteinizing hormone, which is released by the pituitary in response to hypothalamic GnRH in a negative-feedback loop. In circulation, roughly 40 to 60 percent is tightly bound to sex hormone-binding globulin and most of the remainder loosely to albumin, leaving a small free fraction that, with albumin-bound hormone, is considered bioavailable. At target tissues testosterone binds the intracellular androgen receptor, which translocates to the nucleus and modulates gene transcription governing muscle protein synthesis, bone density, and erythropoiesis. Its actions are amplified or diversified by two conversions: 5-alpha-reductase yields dihydrotestosterone, a more potent androgen important in some tissues, while aromatase converts testosterone to estradiol, which supports bone, libido, and cardiovascular and cognitive function in both sexes. Research associates testosterone with lean mass, strength, libido, mood, and metabolic parameters, and describes age-related decline as gradual and highly individual. Because SHBG shifts with age, insulin, thyroid status, and body composition, free and total measures together are studied as more informative than either alone.
