Estradiol (E2) is the most potent of the body's naturally occurring estrogens and a key signaling hormone in both women and men. It is studied for wide-ranging roles in bone density, mood, cognition, sexual health, and cardiovascular and metabolic physiology.

How It Works

Estradiol, often abbreviated E2, is the strongest of the three main estrogens the body makes, and it acts as a chemical messenger that travels through the bloodstream to influence tissues throughout the body. In women it is produced mainly by the ovaries and rises and falls across the menstrual cycle, while in men and in postmenopausal women it is made largely by converting testosterone in fat, brain, bone, and other tissues. Although it is often thought of as a female hormone, both sexes rely on it. Estradiol works by fitting into specific receptors inside cells, where it switches certain genes on or off, and it is associated with bone strength, mood and cognition, skin and vaginal health, and aspects of heart and metabolic function.

The Science & Mechanism

Estradiol is a C18 steroid synthesized from cholesterol through pregnenolone and androgen intermediates, with the enzyme aromatase (CYP19A1) converting testosterone to estradiol in the ovaries, testes, adipose tissue, bone, brain, and vascular endothelium. In premenopausal women, ovarian granulosa cells are the dominant source under pituitary FSH and LH control within the hypothalamic-pituitary-gonadal axis; after menopause and in men, peripheral aromatization predominates. Most circulating estradiol is bound to sex hormone-binding globulin and albumin, leaving a small free fraction that is biologically active. It signals through two nuclear estrogen receptors, ERalpha and ERbeta, which act as ligand-activated transcription factors binding estrogen-response elements to modulate gene expression, and through membrane-associated GPER for rapid, non-genomic effects on signaling cascades. Research describes estradiol restraining bone resorption by promoting osteoclast apoptosis and supporting osteoblasts, modulating neurotransmitter and neurotrophic systems relevant to mood and cognition, influencing endothelial nitric oxide and lipid handling, and shaping reproductive tissue. Its actions are highly tissue-specific, and it is metabolized in the liver to estrone and further conjugated for excretion.
