How It Works
The HPA axis is a communication loop between three points: the hypothalamus and pituitary in the brain, and the adrenal glands atop the kidneys. When you perceive a stressor, the hypothalamus signals the pituitary, which signals the adrenals to release cortisol, the body's main stress hormone. Cortisol then helps mobilize energy, sharpen focus and modulate inflammation, and it feeds back to the brain to switch the response off once the stressor passes. In everyday terms, it is the system that ramps you up under pressure and, ideally, brings you back to baseline afterward. When this rhythm is well regulated, cortisol follows a natural daily curve, higher in the morning and lower at night.
The Science & Mechanism
The HPA axis operates through a signaling cascade. The paraventricular nucleus of the hypothalamus releases corticotropin-releasing hormone (CRH), which prompts the anterior pituitary to secrete adrenocorticotropic hormone (ACTH). ACTH travels through the bloodstream to the adrenal cortex, stimulating the zona fasciculata to synthesize and release cortisol, a glucocorticoid. Cortisol acts on glucocorticoid and mineralocorticoid receptors distributed throughout the body, influencing glucose metabolism, immune signaling, vascular tone and central nervous system activity. A defining feature is negative feedback: circulating cortisol binds receptors in the hippocampus, hypothalamus and pituitary, suppressing further CRH and ACTH release and returning the system toward baseline. Cortisol secretion also follows a circadian rhythm driven by the suprachiasmatic nucleus, typically peaking shortly after waking (the cortisol awakening response) and declining across the day. Research describes how chronic or repeated stressors are associated with altered feedback sensitivity and a flattened or shifted cortisol curve. Studies explore the interplay of the HPA axis with the autonomic nervous system, the gut-brain axis and other endocrine loops as part of the broader stress-adaptation network.
