Small intestinal bacterial overgrowth describes an excess or abnormal type of bacteria colonizing the small intestine, where numbers are normally low. Research links this imbalance to bloating, gas, altered bowel habits and, over time, effects on nutrient status and whole-body wellbeing.

Overview

Small intestinal bacterial overgrowth (SIBO) is a condition in which bacteria accumulate in the small intestine in greater numbers, or of different species, than typically belong there. The small bowel is usually kept relatively sparse of microbes by stomach acid, bile, digestive enzymes, cleansing muscular waves and the ileocecal valve, so overgrowth often reflects a breakdown in one of these protective systems. It is associated with symptoms such as bloating, excess gas, abdominal discomfort, and changes in bowel habits, and it appears more often in people with conditions that slow gut motility or alter anatomy. Viewed through a full-body optimization lens, SIBO matters because bacteria fermenting food high in the digestive tract can interfere with the absorption of vitamins, fats and other nutrients, subtly shaping energy, mood and long-term resilience beyond the gut itself.

The Underlying Biology

The small intestine normally hosts far fewer bacteria than the colon, a balance maintained by gastric acid, bile acids, pancreatic enzymes, intestinal immunity, the ileocecal valve and the migrating motor complex, the rhythmic sweeping contractions that clear residue between meals. When these defenses falter, bacteria proliferate and ferment carbohydrates prematurely in the small bowel, producing hydrogen, and in some cases hydrogen sulfide, gas. Certain archaea, chiefly Methanobrevibacter smithii, consume hydrogen and release methane, a pattern often linked with constipation and now described as intestinal methanogen overgrowth. This fermentation drives distension, bloating and altered motility. Bacterial deconjugation of bile acids can impair fat and fat-soluble vitamin absorption, while microbial competition and mucosal changes may lower vitamin B12 yet sometimes raise folate. Risk is associated with impaired motility, prior abdominal surgery, structural changes, low stomach acid, and conditions such as diabetes, scleroderma and hypothyroidism. Commonly assessed markers include breath testing that measures hydrogen and methane after a glucose or lactulose challenge, alongside evaluation of B12, iron, folate and fat-soluble vitamin status.
