Overview
Diverticular disease is an umbrella term for small, sac-like outpouchings called diverticula that push through weak points in the wall of the large intestine, most often the sigmoid colon. The simple presence of these pouches is called diverticulosis, which is extremely common and usually silent, becoming more frequent with each decade of life so that a majority of older adults in Western countries carry them. In a subset of people the pouches become inflamed or infected, a state called diverticulitis, or drive chronic symptoms and bleeding. Viewed through a full-body optimization lens, diverticular disease sits at the intersection of dietary fiber intake, colonic motility, the gut microbiome and low-grade inflammation, so it reflects broader patterns in how the digestive system and the body age rather than an isolated plumbing problem.
The Underlying Biology
Diverticula form where the colon wall is structurally weakest, at points where blood vessels penetrate the circular muscle layer. Long-standing models emphasize elevated intraluminal pressure: low stool bulk, often linked to low fiber intake, is thought to require stronger, more segmenting colonic contractions that raise pressure and herniate the mucosa and submucosa outward through these gaps. Age-related changes in the wall itself contribute, including altered collagen and elastin cross-linking and increased connective-tissue stiffness, which reduce the wall's resistance to pressure. Beyond mechanics, contemporary research explores colonic dysmotility, visceral hypersensitivity, disturbances in the enteric nervous system and shifts in the gut microbiome that may sustain a state of chronic low-grade mucosal inflammation. When acute diverticulitis develops, micro-perforation and bacterial translocation trigger a localized inflammatory and immune response that can range from mild wall thickening to abscess or perforation. Bleeding arises when the vessels arching over a diverticulum are exposed and rupture. Commonly assessed markers and tools include colonoscopy or CT imaging for diagnosis, and inflammatory readouts such as C-reactive protein, white blood cell count and fecal calprotectin during acute episodes, alongside fiber intake and dietary patterns.
