How It Works
Vitamin D is unusual among nutrients because your body treats it less like a vitamin and more like a hormone. Your skin makes it when exposed to sunlight, and it also comes from a few foods and supplements, but in either case it must be activated by the liver and kidneys before it can act. Once activated, it travels through the bloodstream and switches genes on and off inside cells throughout the body, including immune cells. The Science & Mechanism
Vitamin D exists chiefly as D3 (cholecalciferol), made in skin from 7-dehydrocholesterol under UVB light, and D2 (ergocalciferol) from some plant and fungal sources. It is biologically inactive until hydroxylated twice: first in the liver to 25-hydroxyvitamin D, the circulating form measured clinically, and then in the kidney and many peripheral tissues to the active hormone 1,25-dihydroxyvitamin D (calcitriol). Calcitriol binds the vitamin D receptor (VDR), a nuclear receptor that partners with the retinoid X receptor and regulates transcription of hundreds of genes. VDR is expressed not only in intestine, bone, and kidney but also in monocytes, macrophages, dendritic cells, and activated T and B lymphocytes, giving vitamin D a plausible role in immune signaling. Research describes several immune-relevant actions: induction of the antimicrobial peptide cathelicidin, promotion of a more tolerogenic dendritic-cell phenotype, a shift in T-helper balance associated with dampened Th1 and Th17 activity and supported regulatory T cells, and modulation of pro-inflammatory cytokines such as TNF-alpha and IL-6. Beyond immunity, VDR signaling governs calcium and phosphate handling central to bone mineralization.
