Specialized pro-resolving mediators, often shortened to SPMs, are a family of tiny signaling molecules the body builds from essential fatty acids. In everyday terms, inflammation is meant to be a temporary response: it flares up to deal with an injury or infection, then it is supposed to be actively turned off. SPMs are the messengers that carry that turn-off signal. Rather than simply suppressing inflammation the way a brake pedal would, research describes them as an active program that tells immune cells to stop recruiting reinforcements, clean up cellular debris, and return the tissue to a calm, healed state. When this resolution program is sluggish, inflammation can linger in the background, which is the state longevity medicine pays close attention to.
The Science & Mechanism
SPMs are lipid mediators enzymatically derived from long-chain polyunsaturated fatty acids: resolvins of the E-series from EPA, and resolvins of the D-series, protectins, and maresins from DHA, alongside lipoxins made from the omega-6 arachidonic acid. Their biosynthesis proceeds through lipoxygenase (5-, 12-, and 15-LOX) and, in a phenomenon called transcellular biosynthesis, cyclooxygenase-2 acting across neighboring cell types such as neutrophils, macrophages, and platelets. Research characterizes SPMs as agonists at specific G-protein-coupled receptors, including ALX/FPR2, GPR32 (DRV1), ChemR23 (ERV1/CMKLR1), and GPR18 (DRV2), rather than acting as generic anti-inflammatory brakes. Through these receptors they are described as limiting neutrophil infiltration, stimulating macrophage efferocytosis (the clearance of apoptotic cells and debris), promoting a shift toward a reparative macrophage phenotype, and dampening pro-inflammatory cytokine signaling. This defines resolution as an active, receptor-mediated process distinct from passive fade-out. Studies associate deficient SPM production, or an unfavorable ratio of pro-inflammatory to pro-resolving mediators, with persistent low-grade inflammation. Ongoing work continues to characterize how substrate availability, enzyme activity, and receptor expression shape an individual's capacity to resolve inflammation.
