How It Works
ESR is one of the oldest and simplest laboratory tests still in everyday use. When a sample of blood is placed upright in a thin tube, the red blood cells slowly sink under gravity, and the test records how far they fall in one hour. In everyday terms, it is a bit like watching sediment settle in a glass of water: normally the cells drift down slowly, but during inflammation certain sticky proteins coat them so they clump together and fall faster. That faster fall shows up as a higher ESR value. Because it responds to inflammation only indirectly and changes gradually, Dr. Rob reads ESR as one broad signal within a larger picture rather than a diagnosis on its own.
The Science & Mechanism
The sedimentation rate is governed by the physics of how red cells aggregate in plasma. Normally, red blood cells carry a net negative surface charge (the zeta potential) that makes them repel one another and remain suspended. During an acute-phase inflammatory response, the liver, driven largely by interleukin-6 and other cytokines, increases production of plasma proteins such as fibrinogen, along with immunoglobulins and other globulins. These asymmetric, positively interacting proteins neutralize surface charge and bridge cells together, promoting rouleaux formation, stacks of red cells that are denser and settle more rapidly under gravity. The distance the cells fall in one hour, read in millimeters by the Westergren reference method, is the ESR. Fibrinogen is the dominant driver, which is why ESR rises and falls slowly, tracking the days-to-weeks kinetics of these proteins rather than sudden changes. This makes ESR a nonspecific marker: it reflects the presence and rough magnitude of systemic inflammation but not its cause. Values are also influenced by red cell number and shape, plasma composition, age, sex, and pregnancy, which is why research and clinical guidance emphasize interpreting ESR in context.
