How It Works

Between meals, when your digestive tract is empty, it does not simply go quiet. Instead, a slow, organized wave of muscle contractions begins in the stomach and travels down the small intestine, sweeping along leftover food particles, digestive juices, and bacteria toward the large intestine. This recurring pattern is the migrating motor complex, and researchers often nickname it the intestinal housekeeper. It cycles roughly every 90 to 120 minutes while you are fasting and is interrupted each time you eat. The Science & Mechanism

The migrating motor complex is a cyclical pattern of electromechanical activity that recurs approximately every 90 to 120 minutes during the interdigestive (fasting) state and is abolished by eating. It is classically described in phases: phase I, a period of relative quiescence; phase II, irregular intermittent contractions; and phase III, a burst of regular, high-amplitude contractions at maximal frequency that migrates aborally from the stomach and proximal small intestine. Phase III is considered the key housekeeping component, propelling residual chyme, sloughed cells, and bacteria distally. Regulation is both hormonal and neural. The peptide hormone motilin, secreted by enteroendocrine M cells in the duodenum and proximal jejunum, is cyclically released in the fasted state and is closely associated with the initiation of gastric phase III. The related hormone ghrelin and the vagus nerve and enteric nervous system also contribute to patterning and propagation. Research explores the MMC's role in limiting bacterial accumulation in the small bowel, and altered or absent MMC activity is investigated in relation to small intestinal bacterial overgrowth and functional gut symptoms.
