The ileocecal valve is a one-way-style gateway sitting where the last segment of the small intestine (the ileum) empties into the first part of the large intestine (the cecum). In everyday terms, it works like a traffic gate on a busy road: it opens to let digested material pass forward into the colon, then closes to hold that traffic from flowing backward. This timing gives the small intestine enough contact with food to finish absorbing nutrients and water before contents move on. Just as importantly, the closed valve helps keep the dense bacterial population of the colon from drifting up into the relatively bacteria-sparse small intestine. The ileocecal valve, or ileocecal sphincter, is a specialized zone of thickened circular smooth muscle at the ileocolonic junction, formed by two mucosal lips anchored by frenula that project into the cecal lumen. It maintains a tonic resting pressure, reported around 20 cm H2O over roughly 4 cm, that creates a physiologic barrier between ileal and colonic contents. This tone is dynamically modulated: distension of the ileum reflexively relaxes the sphincter to permit forward flow of chyme, while distension of the cecum contracts it, resisting retrograde reflux of colonic bacteria and material. The enteric nervous system coordinates these responses locally, with vagal parasympathetic input associated with relaxation during ileal peristalsis and sympathetic, alpha-adrenergic signaling associated with contraction on colonic distension. The gastroileal reflex, triggered by gastric filling, is described as intensifying ileal peristalsis and promoting valve opening so residual chyme advances into the colon.
